How Do We Ensure Continuity Of Care Between Home Care Staff?

In order to ensure that we continue to provide the level of care that we’ve become renowned for over the last two decades, all relevant patient information is shared between different teams and healthcare services as appropriate, on an ongoing basis.

Collaboration is key

Depending on the health conditions of our clients, there may well be many different people involved in care provision. 

This doesn’t just include the private nurses and live-in carers that make up the Concept Care family, but also other external healthcare providers including doctors, nurses, specialists, physiotherapists, counsellors and so on.

With so many individuals working on one case, there is the potential for standards to slip, but we make sure that all information is readily available to all relevant personnel, so that they can assess, plan and deliver care, treatment and support as appropriate.

We understand that ensuring continuity of care can have a huge impact on the wellbeing of our clients and their quality of life. In fact, studies go so far as to show this continuity can have a positive impact on patient satisfaction, decrease hospitalisations and emergency room visits, and improve receipt of preventive services.

Concept Care at work

Practically speaking, this means creating and maintaining a comprehensive care path for everyone we have the pleasure and privilege of working with. 

In order to accomplish this, we first have to build up an in-depth understanding of the personal histories, cultures, needs and preferences of our clients so that we can tailor our care provision appropriately.

The key to success is building long-term meaningful relationships with you and your loved ones so that we can create a culture of trust and familiarity, with a person-centred approach to care at the heart of all we do.

We get to know all our clients as individuals, learning all we can about them, their stories, their interests and what their daily routines are like. 

From there, we make sure that they’re fully involved in the development of their care plan, helping to give them a sense of control over what happens.

This isn’t a one-off activity, however, and we carry out regular care plan reviews to ensure that our service provision continues to be what you need, with details evolving in line with changing health requirements over time.

As and when care transitions have to take place, we prioritise communication and collaboration, ensuring that the high standards of care continue to be delivered with uninterrupted support through the use of consistent documentation, clear and easy-to-understand notes, and regular check-ins between team members.
